 This fillable .pdf form includes necessary school, coach and team member information - e-mail this to the BCB Coordinator! The Coach signs an agreement regarding guidelines for participation and submits a participation fee - mail this to the BCB Coordinator.

(1) Consent for student to participate in NOSB competitions, regional and/or national; (2) Student medical/emergency forms for BCB and NOSB Coordinator use only; (3) Consent for student to be interviewed and photographed, acknowledge NOSB right to publish.
